About The Position

The Mental Health Counselor
is responsible for providing psychosocial assessments, collaborative treatment planning, engagement, referrals, and evidenced-based clinical interventions as indicated for individuals dealing with a variety of mental health and co-occurring substance use issues.

Requirements

• Masters of Social Work or other related field with two (2) years of related experience
• LMSW or LMHC required, LCSW preferred
• Candidate must be able to communicate clearly and professionally in writing and verbally, in English (with additional Spanish-speaking strongly preferred)
• Demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills
• Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
• Knowledge of Diagnostic and Statistical Manual of Mental Disorders
• Working knowledge utilizing electronic medical records software (Medent) strongly preferred.

Responsibilities

• Conducts initial psychosocial assessments and provides initial treatment recommendations, and collaboratively with the patient develops a personalized treatment plan with emphasis on evidence-based practice within a harm reduction orientation
• Provides resources and referrals as indicated to facilitate achievement of a patient’s treatment goals
• Provides psychotherapy and crisis intervention to individuals and families who are dealing with a variety of issues, including but not limited to mental health, co-occurring substance use disorders, chronic medical conditions, including HIV, HCV, and issues related to sexual orientation and gender identity
• Adheres to program expectations and requirements surrounding productivity, documentation timelines and standards, and overall quality of patient care
• Works within an integrated treatment model, coordinating treatment with both internal and external providers as indicated
