About The Position

The Mental Health Clinician in the CARES Program at Trinitas Regional Medical Center is responsible for responding to crisis calls, conducting initial assessments, and developing treatment plans for assigned caseloads. This role involves providing ongoing training and consultation to assist provider agencies and families in managing behavioral and psychiatric issues, ensuring crisis stabilization, and maintaining patients in their community placements.

Requirements

  • Master's degree in Social Work, Counseling, Psychology, or a related field.
  • Minimum one year of experience working with children/adolescents or youth and families.

Responsibilities

  • Respond to crisis calls from screening centers and provide immediate service.
  • Conduct initial assessments face-to-face according to the Crisis Response Protocol.
  • Develop treatment plans for assigned caseloads and provide ongoing clinical case management.
  • Consult with intake clinicians to gather initial information on client presentations.
  • Facilitate access to Trinitas MI/DD Psychiatric Inpatient Unit as needed.
  • Work with clients at screening centers to maintain behaviors and develop discharge plans.
  • Follow up with discharged clients in their community settings to provide ongoing consultation and support.
  • Maintain contact with individuals, provider agencies, and families to manage maladaptive behaviors.
  • Perform on-site coaching in community settings and conduct training for agencies.
  • Participate in inter-agency case conferences and interdisciplinary team meetings.
  • Provide clinical consultation to other service providers and respond to off-hours crisis calls as directed.
  • Document assessments and interventions in the CARE-DD electronic medical record system promptly.
  • Assist in obtaining services and resources for individuals, including mental health and DDD services.
  • Complete weekly statistical reporting on activities and participate in staff meetings.
