The Licensed Clinical Social Worker (LCSW) will be responsible for providing brief, evidence-based psychotherapies and supportive counseling to a select primary care clinic population. Treatment will be provided in-person at the CU Medicine Family Medicine – Centerfield clinic. The Licensed Clinical Social Worker will provide short-term psychotherapy across the continuum of care, including identifying eligible patients based on defined criteria, providing psychosocial assessment, developing and executing treatment in alliance with patient’s goals, and ultimately ending care with a firm discharge plan in place. In addition, this role will be responsible for providing crisis support to clinic patients identified as needing urgent behavioral health needs and ensuring they are provided with and/or connected to the next appropriate level of care. In addition to providing brief psychotherapeutic interventions, this role incorporates social work–informed care management activities, including assessment of psychosocial needs, addressing social determinants of health, coordinating referrals, and supporting patient navigation across healthcare and community-based services to reduce barriers to care. The Licensed Clinical Social Worker will work in collaboration with the behavioral health team as well as the primary care clinic’s multidisciplinary team including the patient’s primary care provider and clinic leadership.
