About The Position

POSITION PURPOSE: Mental Health Clinician - BPSR provides clinical care coordination, rehabilitation and recovery services for adult clients, ages 18+ with SMIs on an outpatient basis, including adults with a COD; and performs related work required.

Requirements

  • Master’s degree and higher (waived PsyD or PhD) in social work, marriage and family therapy, or counseling with graduate level internship experience with SMI clients or 1-2 years post master’s experience working with SMI including those with COD or an equivalent combination of training and experience.
  • Must be registered with the Board of Behavioral Science (BBS) Examiners; or is license waivered by State of California, or eligible to register with the BBS Examiners
  • A Valid Driver’s License with current California automobile insurance
  • A Valid Cardiopulmonary Resuscitation (CPR) Certification
  • A Valid Tuberculosis Blood (TB) test

Nice To Haves

  • One Crisis Intervention Specialist CC and one CC position must be Bilingual in Spanish.
  • Bilingual preferred.
  • Bilingual candidates will be required to pass a bilingual competency test prior to their hire date.
  • On occasion may require transportation of clients in personal vehicle.

Responsibilities

  • Provides ongoing multi-disciplinary care coordination, biopsychosocial assessments, individual/group counseling, case management, and rehabilitative support services to all clients;
  • Works closely with Healing Oaks Clinic SUD Program to ensure continuity of care for clients dually enrolled;
  • Works with clients to develop service plans, specifying goals/objectives, types of services, and therapeutic interventions;
  • Performs crisis intervention as needed;
  • Assesses/identifies clients’ needs for supportive services on an ongoing basis;
  • Serves as a resource, performing case management services by assisting in obtaining, and/or makes appropriate referrals for necessary or requested services (e.g., housing, vocational, employment, food) follows up with referral party to ensure client connected to services;
  • Collaborates with other team members on client concerns;
  • Attends, participates in and provides information about assigned clients in service review and care coordination meetings/consultations;
  • Requests assistance/guidance from management/professional staff when necessary to address client issues/problems;
  • Establishes/maintains contact with community resources to ensure continuity of care (e.g., case managers, conservators);
  • Creates detailed documentation of client progress using an EHR system;
  • Maintains client charts/care coordination databases;
  • Updates assigned clients’ progress notes;
  • Maintains daily log of clients seen;
  • Writes necessary communication (e.g., letters for court appearances);
  • Completes pre-assessments, intake and updated assessments, and other necessary paperwork, including service plans, updates, and reviews charts;
  • Enters necessary data/information in appropriate files/systems;
  • Will attend community events and trainings related to the delivery of specialized EBPs for the target population;
  • Other duties as assigned.
