Provides direct delirium prevention/intervention services for patients admitted on the medical floors of the hospital as part of the Hospital Elder Life Program. Principal Accountabilities: Provide beside evidenced based delirium prevention interventions for patients throughout the hospital, including recall, recollection and memory engagement techniques. Collaborate with nursing, social work and medical providers in coordination with HELP program coordinator to provide coordinated delirium prevention/intervention needs. Orientate and educate patients and care givers/family about the HELP program. Complete education and orientation needs for HELP volunteers in coordination with the HELP Clinicians, Coordinator, and Service Line Manager. Participate and complete shadowing and rounding with team members and HELP volunteers in coordination with the HELP Coordinator and Service Line Manager. Attend service line, departmental and site-specific performance improvement and quality meetings in coordination with the HELP Coordinator and Service Line Manager. Complete all required documentation accurately and within the timeframes required by the department. Assist the HELP Coordinator and Service Line Manager with program related analytics and reporting build needs. Collaborate with the Psychiatric Consult Liaison team in coordination with the HELP Clinicians, Coordinator, and Service Line Manager. Attend, organize, and facilitate departmental education and program related events in coordination with the HELP Clinicians, Coordinator and Service Line Manager. Coordinate with the hospital volunteer services team to recruit, onboard, educate, and orientate HELP volunteers. Other responsibilities as assigned.
