About The Position

A professional registered nurse (RN) who offers individualized assistance to patients, families and caregivers to help overcome health care system barriers. Enhances the patient experience and improves the clinical outcome for patients by providing a seamless navigation process throughout the continuum of care. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families. Participates in strategic initiatives within the clinical program or service line to include developing, implementing and monitoring navigation activities and sustainability, facilitates shared decision making and patient engagement by forming trusting relationships and addressing patients' communication and health literacy needs, while helping them integrate their values and preferences into treatment decisions. Works for and promotes positive outcomes for patients, providers and the Sutter health care system. The RN – Memory Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients from the beginning of their journey to proactively identify and resolve needs and ensure all patients have immediate access to emotional support and clinical guidance throughout their care within the Sutter Health System. The RN - Memory Navigator works in collaboration with the care team and other multidisciplinary teams to establish and coordinate a plan of care across the system for Sutter Health patients. The RN - Memory Navigator systematically and continuously collects and assesses information related to patient health status to develop, execute and evaluate the plan of care for patients and/or family. The RN - Memory Navigator must communicate effectively with faculty, staff, and external contacts using tact, discretion and diplomacy. This person must possess well developed problem solving and team development techniques. Key responsibilities include direct patient care, program planning and evaluation, participation in staff development and education, and quality improvement initiatives. In addition, a strong working knowledge of the specific disease care standards and core competencies is critical. The RN - Memory Navigator participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

Requirements

  • Graduate of an accredited school of nursing.
  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider
  • 5 years recent relevant experience.
  • Knowledge of medical terminology required.
  • Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff.
  • Verbal and written communication skills.
  • Microsoft Office software experience required, including Excel and Word
  • Travel required as needed.
