About The Position

The Memory Care Support Specialist at Brownsburg Meadows is responsible for leading the Memory Care Team and providing compassionate, person-centered care to residents with dementia and Alzheimer's. This role emphasizes the importance of relationships and accountability in delivering high-quality care, while also supporting the development and implementation of care plans in collaboration with an interdisciplinary team.

Requirements

• Bachelor's degree required.
• Minimum of three years of human services experience, with two years working with dementia or Alzheimer's care/patients preferred.
• Demonstrates C.A.R.E. values to residents, family members, customers, and staff.

Responsibilities

• Maintain and demonstrate a Person-Centered Approach to resident care within the Memory Care Center environment.
• Ensure the delivery of compassionate quality of care to residents.
• Assist the team in managing resident behavioral expressions and interventions through non-pharmacological methods.
• Participate in the development and maintenance of social service and activity portions of the care plan for each resident.
• Educate residents and family members about dementia, answer questions, and resolve concerns.
• Observe and provide staff education on the progression of Alzheimer's disease.
• Interpret and evaluate program policies and procedures.
