Requirements

  • Current and valid license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Wisconsin.
  • Current and valid CPR certification.
  • Basic proficiency in computer use and electronic health record systems.
  • Physically capable of frequently pushing or pulling objects weighing up to 60 pounds, and occasionally lifting or moving objects weighing 100 pounds or more.

Nice To Haves

  • Previous experience in long-term care is preferred.

Responsibilities

  • Administer Medications and Treatments: Safely deliver prescribed medications and treatments in accordance with physician orders, regulatory standards, and organizational policies.
  • Monitor and Document Resident Conditions: Observe residents, accurately document findings, and communicate changes in condition to the appropriate healthcare team members.
  • Support Care Planning and Admission Processes: Collaborate with RNs and interdisciplinary teams in developing, implementing, and updating individualized care plans, including during admissions and readmissions.
  • Participate in and Oversee Resident Care Programs: Actively support resident care initiatives (e.g., skin integrity, pain management) and ensure CNAs fulfill their responsibilities within these programs.
