Requirements

Current and valid license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Wisconsin.
Current and valid CPR certification.
Basic proficiency in computer use and electronic health record systems.
Physically capable of frequently pushing or pulling objects weighing up to 60 pounds, and occasionally lifting or moving objects weighing 100 pounds or more.

Nice To Haves

Previous experience in long-term care is preferred.

Responsibilities

Administer Medications and Treatments: Safely deliver prescribed medications and treatments in accordance with physician orders, regulatory standards, and organizational policies.
Monitor and Document Resident Conditions: Observe residents, accurately document findings, and communicate changes in condition to the appropriate healthcare team members.
Support Care Planning and Admission Processes: Collaborate with RNs and interdisciplinary teams in developing, implementing, and updating individualized care plans, including during admissions and readmissions.
Participate in and Oversee Resident Care Programs: Actively support resident care initiatives (e.g., skin integrity, pain management) and ensure CNAs fulfill their responsibilities within these programs.
