About The Position

The Memory Care Coordinator is responsible for the overall leadership, operation, and quality of care within the Memory Care neighborhood. This role ensures a safe, supportive, and engaging environment for residents while providing direct supervision, training, and support to medication technicians and caregivers. The Memory Care Coordinator works hands-on as needed and ensures compliance with all state Personal Care Home (PCH) regulations and community standards.

Requirements

  • Must be a licensed LPN in the state of Pennsylvania
  • Experience working in memory care, personal care, or a related senior living or healthcare environment
  • Knowledge of state Personal Care Home (PCH) regulations and memory care best practices
  • Strong leadership, communication, and organizational skills
  • Ability to work hands-on with residents and staff in a fast-paced care environment
  • Commitment to resident safety, dignity, and quality of life
  • Ability to work on the floor, assist residents as needed, and respond to emergencies
  • Available for on-call every other weekend

Nice To Haves

  • Experience completing or overseeing resident assessments preferred

Responsibilities

  • Provide day-to-day leadership of the Memory Care neighborhood, including supervision, training, coaching, and discipline of medication technicians and caregivers
  • Monitor residents continuously for changes in health status, behavior, and overall well-being, communicating concerns appropriately
  • Work on the floor as needed to support staffing, resident care, and unit operations
  • Maintain knowledge of state PCH regulations and ensure all guidelines are consistently followed within the unit
  • Complete and oversee required resident evaluations and assessments, including RASP/DME, Level of Care (LOC), and other applicable assessments
  • Ensure shower schedules, laundry services, and personal care routines are followed consistently
  • Develop, maintain, and implement a monthly activities calendar specific to the Memory Care neighborhood
  • Lead and participate in unit-based activities, including structured programming, arts and crafts, and creative engagement opportunities
  • Promote resident-centered care and meaningful engagement tailored to cognitive abilities
  • Ensure a safe, secure, and supportive environment for both residents and staff
  • Collaborate with nursing, administration, and other departments to ensure high-quality care and service delivery
