What You'll Do The Memory Care Coordinator is a key member of the leadership team. This person monitors, trains, and coordinates the memory care community's programming, staffing, and care services. Responsibilities include the coordination of Life Enrichment activities and training, assisting the Health Services Coordinator with assessments and service plans for each elder; Collaborating with the Life Enrichment coordinator regarding activities; facilitating a monthly family/resident council; and hosting a monthly Alzheimer's support group or Lunch and Learn for families within the community, and in the greater community. As a Memory Coordinator, you can expect a range of responsibilities and tasks. Here are some general expectations: Hires, trains, supervises, motivates, and evaluates caregiver staff to ensure residents' highest quality of care. Works directly with the Wellness Team and Administrator to plan and assign work duties and assignments for the Care Partners. Monitors the day-to-day work provided by the Care Partners, supervising, mentoring, suggesting, and educating the staff on proper work standards. Monitors and coordinates the housekeeping duties with the housekeeping department. Perform the duties of a medication aide or Care Partner when needed. Completes initial paperwork on all new residents and routinely audits elder charts to ensure paperwork is complete and filed correctly. Monitors and implements the service plan process, including the interim service plan. This includes updating service plans on a routine and timely basis and scheduling and facilitating service plan meetings quarterly with elders, team members, and families.
