About The Position

The Memory Care Coordinator (Social Worker) at Aspen Trace Cardon & Associates is responsible for overseeing the Cherished Memories Alzheimer's and dementia care unit. This role involves managing various functions within the unit, ensuring quality care for residents, and providing support to staff and families. The coordinator will conduct assessments, manage schedules, and handle concerns related to memory care, while also facilitating training and maintaining necessary records.

Requirements

• Experience in Alzheimer's care and long-term care management.
• Knowledgeable about dementia illnesses and their impact on individual functioning.
• Ability to read, write, and follow written and oral instructions in English.
• Previous management experience in long-term care preferred.

Nice To Haves

• Social Work/Social Services Director experience with Alzheimer's care.
• Knowledge of adaptive strategies to maintain resident abilities.

Responsibilities

• Oversee all functions of the Cherished Memories Alzheimer's and dementia care unit.
• Conduct Functional Needs Assessments.
• Perform budget analysis, review, and control.
• Manage and update emergency packets and appointment books.
• Provide support to frontline staff as needed.
• Handle all Memory Care Unit family and resident concerns, questions, and feedback.
• Conduct scheduled InService meetings and trainings for Memory Care Unit staff.
• Oversee dining services in the Memory Care Unit.
• Implement behavioral interventions and problem-solving strategies.
• Maintain records of resident assessments and ensure timely completion.
