About The Position

This position plays a key role in advancing member health outcomes through strategic outreach, education, and engagement initiatives. This position is responsible for independently conducting outreach to health plan members via phone, email, and written communication to promote preventive care, chronic condition management, and overall wellness. The MEC collaborates cross-functionally with internal teams, analyzes member data to inform on outreach strategies, and contributes to care coordination efforts with a focus on continuous improvement and member satisfaction. The MEC will strengthen member relationships, improve education and outreach. The MEC will engage members to encourage gap closures, promote preventive care visits, (such as scheduling colonoscopies, mammograms, and other screenings), and coordinate with PCP offices to increase completion of annual wellness visits and routine physical exams.

Requirements

  • High school diploma or general education degree (GED) required. Associate’s or Bachelor’s degree in health education, public health, communications, or a related field is preferred.
  • Minimum of 1–2 years of experience in member engagement, care coordination, community health, or customer service in a healthcare setting required.
  • Valid Oregon Driver’s License and vehicle insurance.
  • Current Verbal De-escalation (CPI) Certification or willingness to be certified within 90 days of hire.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
  • Experience with EZ Cap, Essette, or similar healthcare documentation systems.
  • Strong understanding of HIPAA regulations and compliance.
  • Ability to interpret and apply healthcare data to outreach strategies.
  • Familiarity with service recovery and customer experience best practices.

  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
  • Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to work with problems involving several concrete variables in standardized situations.
  • The employee must be able to work onsite, as needed.
  • The employee must be able to travel locally, as needed.
  • Because of exposure to patient records of all types, the highest standard of patient confidentiality and privacy as established by business policy and HIPAA requirements must be maintained.

Responsibilities

  • Designs and executes targeted outreach campaigns to educate members on preventive health services, follow-up care, and wellness resources.
  • Documents member interactions and engagement outcomes accurately in EZ Cap and Essette systems, ensuring compliance with organizational and regulatory standards.
  • Collaborates with clinical, quality, and data teams to analyze member data, identify outreach opportunities, and support care coordination strategies.
  • Will have access to technology systems withing the organization with personal health information (PHI).
  • Serves as a liaison between members and internal departments to resolve benefit-related inquiries and health concerns.
  • Participates in and leads community health events, outreach initiatives, and special projects that promote member engagement.
  • Ensures strict adherence to HIPAA privacy and security regulations.
  • Communicates complex health information clearly and empathetically, tailoring messaging to diverse member populations.
  • Manages multiple outreach projects with precision, ensuring timely follow-up and accurate reporting.
  • Assesses member needs during interactions and escalates concerns to appropriate care teams.
  • Identifies trends and opportunities for program improvement using data insights and member feedback.
  • Maintains flexibility to support offsite events, community meetings, and member education sessions.
  • Contributes to data collection, analysis, and reporting to evaluate outreach effectiveness and inform strategic decisions.
  • Maintains punctual, regular and predictable attendance.
  • Works collaboratively in a team environment with a spirit of cooperation.
  • Respectfully takes direction from leadership.
  • Meets all required training including those listed in Relias Learning Module System (LMS).
  • Performs other duties as assigned.
