About The Position

We are looking for a Full-Time Care Partner I (Medication Technician/Caregiver) to support our Memory Care Community. In this role, you will help support residents and fellow coworkers with resident care needs. Important Note: Some Weekend Availability is Required I. General Purpose of the Job Care Partner I position is responsible for assisting assigned residents in all aspects of their daily living. This includes, but is not limited to, ADL’s, meals, activities, passing medications as ordered and directed, treatments, light housekeeping, and managing the day-to-day schedule of their assigned residents. It will also include communication with staff leadership, family, and outside providers. This position works collaboratively with all other Memory Care Community staff on duty, family members, and volunteers to ensure that all resident needs are being met in a person-centered, relationship-based manner. Requirements

  • Must pass a California criminal background clearance.
  • Must possess written and oral communication skills in English.
  • Hold at minimum, a high school diploma or GED.
  • Possess basic computer knowledge
  • Must be in good physical and mental condition, capable of performing assigned tasks; Good health must be verified by a health screening performed by a licensed physician, including a chest X-ray or intradermal test for TB.
  • Must be able to hear, talk, and have a clear vision with or without corrective lenses.
  • Must be able to sit, stand, walk, bend, push, pull, kneel, and move intermittently during working hours and lift or exert force of up to 50 lbs.
  • Must be able to perform in a busy environment, subject to frequent interruptions, subject to hostile and emotionally upset residents and family members, subject to falls, infectious diseases, substances, odors.
  • Must be willing to work beyond normal working hours, and in other positions temporarily, when necessary.

Nice To Haves

  • 1-2 years’ experience with working with the elderly in an assisted living or skilled nursing facility is preferred.

Responsibilities

  • Provide assistance with resident ADL’s, with proper documentation.
  • Observe and report changes in resident’s condition immediately to Community Coordinator and/or Wellness Coordinator.
  • Distribute medications according to the doctor’s orders and follow all procedures and regulations as trained.
  • Remake beds daily, stripping sheets and sending them to laundry on a routine basis.
  • Maintain the Wellness Room with adequate supplies and keep the area neat and organized.
  • Identify safety hazards, emergencies, and change of conditions and report them to the Wellness Coordinator immediately.
  • Document what was done for residents and necessary information in the Communication Book and Matrix progress notes.
  • Obtain urine and stool specimens, weigh residents, and complete treatments and safety checks as assigned.
  • Ensure that all residents are at each meal, are accounted for if not in the dining room at any given meal.
  • Utilize dementia protocols and interventions to engage residents in daily activity and life.
  • Follow through with Physician orders, including (but not limited to) ordering new medications, changing current orders, and ordering labs as needed and as ordered by a Physician.
  • Send a copy of all laboratory and/or test results to a resident’s Physician immediately.
  • Record any resident admissions, discharges, transfers, or those out on pass in the communication book daily.
  • Engage and participate in activities with residents as appropriate.
  • In coordination with other staff members, such as Life Engagement Team members, identify, develop, and offer supplies for activities that can help calm agitated, anxious, or aggressive behaviors.
  • Report all accidents/incidents, regardless of how minor, to the Wellness Coordinator, responsible party, and doctor and document them on the Incident Report form.
  • Participate in orientation and training of new employees.
  • Perform all other related duties as may become necessary or as directed by the Director of Residential Services.

  • Adhere to and implement a person-centered, relationship-based approach to working with residents and their families.
  • Assist in providing and maintaining an environment that provides a balance of structure and nurture, while working to engage residents in daily life, encouraging them to be as independent as possible.
  • Complete and maintain required training requirements related to resident care, dementia care, program philosophy and job description related training.
  • Demonstrate and integrate trainings into daily interventions and interaction with the residents.
  • Perform all assigned duties in accordance with our established policies and procedures, residential care procedures, and our safety rules and regulations.
  • Maintain Resident’s Rights and Confidentiality.
  • Attend required training and in-services as mandated.
  • Assume accountability for data contained in the employees’ handbook.
  • Observe infection control procedures.
