As a Medication Safety Pharmacist within Cleveland Clinic's Inpatient Pharmacy at Main Campus, you will play a pivotal role in supporting 60 nursing units, 80 operating rooms, 4 PACUs and 10 cath labs. In this strategic position, you will review safety event reports (SERS) and develop both proactive and reactive initiatives to enhance medication safety for the adult patient population. Collaborating on safety, quality and patient experience committees, you will have the opportunity to influence care on a broad scale while ensuring medications are selected, dosed, administered, and dispensed accurately, timely and respectfully. Requirements

  • PharmD or Bachelor's of Science in Pharmacy from an accredited college of Pharmacy.
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience.

Nice To Haves

  • Board Certification.
  • 3-5 years of experience in an inpatient setting.

Responsibilities

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Direct staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.
  • Educate and train students, residents, other caregivers and/or patients.
