About The Position

Are you passionate about making a meaningful difference in patient care? Join our team as a Medication Reconciliation Technician, where you'll play a vital role in ensuring safe and effective treatment for patients from admission through discharge. We are looking for a strong candidate who has day and evening shift availability (plus weekend availability).

Requirements

  • High School Diploma or equivalent.
  • Two or more years of experience in a related role preferred (such as EMT, Paramedic, Pharmacy Technician).
  • Ability to communicate effectively in relation to medications.
  • Will complete a medical terminology course during orientation and corresponding electronic modules related to the job.

Nice To Haves

  • LPN preferred.
  • Prior experience in a healthcare setting (hospital, pharmacy, skilled nursing facility, or medical office) is preferred.
  • Familiarity with medical terminology is a plus.

Responsibilities

  • Engage directly with patients upon arrival to gather and document their home medications.
  • Support the medical team by providing accurate medication histories that inform treatment planning.
  • Collaborate across departments, including the Emergency Department and inpatient units, to reconcile medications throughout the patient's stay.
  • Communicate with pharmacies, families, and care teams to ensure complete and accurate information.
  • Document findings in the electronic medical record and assist with discharge planning to support smooth transitions of care.
  • Collect and verify patients' home medication lists by conducting interviews and gathering information from external sources such as physician offices, family members, pharmacies, and care facilities.
  • Obtain and record vaccine information from patients or prior medical records to support comprehensive care planning.
  • Assist with discharge-related medication needs, including 'meds-to-beds' services and other planning tasks within your scope.
  • Maintain open, respectful communication with team members and supervisors.
  • Uphold all confidentiality policies and manage your workload effectively to meet patient care needs.
