About The Position
The Medication Reconciliation Technician plays a crucial role in ensuring the accuracy of medication lists for patients admitted to Blessing Hospital. This position is responsible for collecting comprehensive information regarding the current home medication list of patients, which includes both prescription and nonprescription medications that the patient has been taking prior to their hospital admission. The technician will gather this information through various means, including direct interviews with the patient or their significant others, reviewing the patient's home medication list, and checking any home medications that the patient may have in their possession. Additionally, the technician will consult long-term care medication administration records, physician clinic lists, and retail pharmacies as necessary to compile a complete and accurate medication list. Once the medication information is collected, the technician will document the current medications and collaborate with the pharmacist to conduct an initial review. This process is vital for ensuring patient safety and effective medication management during their hospital stay. The technician must possess strong communication skills to effectively interact with patients and healthcare professionals, as well as the ability to work with various documentation systems to maintain accurate records. This position requires a commitment to detail and a thorough understanding of medication management protocols.
