POSITION SYNOPSIS: The Medication Nurse plays a key clinical role in ensuring safe, accurate, and effective medication management for Elderhaus PACE participants. This position serves as the primary liaison with pharmacy partners, processes medication orders, oversees medication delivery workflows, and provides participant/caregiver education related to medication use. The Medication Nurse maintains thorough documentation, supports interdisciplinary care planning, and contributes to clinical quality improvement initiatives while ensuring a safe, organized, and participant‑centered environment.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current, unencumbered North Carolina Registered Nurse (RN) license required.
  • Current CPR certification/First Aid certification (certification provided by organization at no cost to employee if hired for role).
  • Minimum 1 year of experience working with a frail or elderly population preferred.
  • Strong communication, assessment, and clinical judgment skills.
  • Ability to collaborate effectively with participants, caregivers, pharmacy partners, and interdisciplinary care team members.
  • Intermediate computer proficiency with comfort navigating EMR systems.
  • Excellent organizational skills with attention to detail and regulatory compliance.
  • Ability to work in a fast‑paced, participant‑focused environment with fluctuating activity levels.
  • Ability to meet physical demands including standing, walking, bending, and occasional lifting.

Nice To Haves

  • Experience in medication management, long‑term care, home health, or PACE strongly preferred.

Responsibilities

  • Receive, enter, and process medication orders daily as the primary point of contact with pharmacy vendors.
  • Administer medications according to Medication Administration policies aligned with RN scope of practice.
  • Collaborate with providers, nurses, participants, and caregivers regarding medication needs, concerns, changes, and adherence support.
  • Coordinate medication pickups and deliveries in partnership with backup pharmacies.
  • Document all medication activities accurately and promptly within the electronic medical record.
  • Verify medication deliveries, ensure proper handling, and maintain chain‑of‑custody procedures for controlled substances.
  • Assist with QAPI (Quality Assurance & Performance Improvement) initiatives, including data collection and process improvement activities.
  • Conduct periodic audits of medication storage areas, medication carts, and emergency medication kits.
  • Prepare medications for return, disposal, or community partner use when appropriate.
  • Participate in IDT meetings, clinic coordination activities, and the on‑call nursing rotation for after‑hours support.
  • Assist with medication reconciliation, enrollment‑day medication counts, and preparation of participant medication planners.
  • Follow infection control guidelines, universal precautions, and hazardous waste policies to maintain a safe and compliant care environment.
