We are looking for a Medication Management NP/PA to join our team in Whittier and provide specialized chronic disease care in collaboration with primary care providers and specialists. Position Summary: PIH Health Medication Management is a specialized subspecialty focused on comprehensive chronic disease management, working in collaboration with primary care providers and specialists across PIH Health Group and affiliated practices. The Nurse Practitioner or Physician Assistant works independently to provide care for patients with acute and complex chronic conditions. This role includes performing and documenting comprehensive physical examinations, administering preventive care, conducting chronic illness surveillance, and assessing, diagnosing, and managing health-illness needs specific to conditions supported by the Medication Management clinic.
