About The Position

The Medication Management Clinical (MMC) Pharmacist is responsible for providing pharmaceutical care services in an ambulatory care setting, focusing on customer-centered clinical pharmaceutical care, including primary care, anticoagulation services, and chronic condition management. The role involves collaboration with a multidisciplinary team to ensure medication safety and effective patient outcomes, while also serving as a medication information specialist and participating in educational programs for patients and providers.

Requirements

  • PharmD degree from an accredited school of pharmacy recognized by the California State Board of Pharmacy.
  • California Registered Pharmacist license.
  • Current BCLS Healthcare Provider CPR Card from the American Heart Association.
  • Completion of an ambulatory care residency and/or advanced certification in a pharmacy specialty area.
  • Minimum of three (3) years working experience as a California Registered Pharmacist in a hospital or ambulatory care setting with clinical background in a specialty area.

Nice To Haves

  • Experience with design of pharmacy systems and equipment specific to medication administration.
  • Bilingual proficiency.

Responsibilities

  • Provide pharmaceutical care services in the ambulatory care setting utilizing approved protocols and established policies.
  • Deliver customer-focused clinical pharmaceutical care including primary care, anticoagulation services, and chronic condition management.
  • Collaborate with the multidisciplinary team to identify clinical interventions and ensure medication safety.
  • Serve as a medication information specialist by disseminating pertinent information to physicians, patients, nursing, and other clinical staff.
  • Participate in patient and provider educational programs to enhance patient compliance and provider prescribing practices.
  • Monitor patients' drug therapies and accept responsibility for outcomes.
  • Maintain the security of clinic medication supplies.
  • Utilize pharmacy technology systems to improve patient safety and streamline workflow.
  • Prepare quality reports, medication utilization evaluations, and participate in performance improvement activities.
