The Pharmacy Medication History Technician serves as a key and integral part of the progressive pharmacy-managed medication reconciliation practice model. This position reports to the Pharmacy Manager under the supervision of a pharmacist and the direction of the Medication History Pharmacy Technician Group Leader. The primary responsibility is to accurately obtain and compile current detailed and complete home medication history lists and allergies for patients to be admitted to the hospital. The technician uses all relevant sources to obtain the most current required information, including patient and family interviews, direct contact with outpatient pharmacies, extended care facilities, and provider offices. The technician also documents detailed and verified current home medication lists in the electronic medical record (EMR) and reviews patient allergy information, updating as needed. Effective and compassionate interaction with patients, families, and other healthcare providers is essential, as well as participation in continuous quality improvement activities.
