About The Position

Corewell Health is offering up to $2,500 as a Sign-On Bonus for this opportunity! (for external candidates or for students graduating Corewell Health's Pharmacy Technician Student Program) About Pharmacy Medication History In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient's location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution. Scope of Work Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist. Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system. Clarifies information through communication with providers, patients, and pharmacies. Performs special projects for the department. Works with multiple medication automation systems. Provides professional customer service as a liaison between the pharmacy department and patients/caregivers. Interacts with prescriber as needed. Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles. Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist. Checks and verifies accuracy of patient profiles. Maintains clean and organized work area. Maintains all requirements for CPhT certifications. Completes all Spectrum Health assigned competencies by assigned due dates. Requirements

  • Required High School Diploma or equivalent
  • 1 year of relevant experience Pharmacy Required
  • LIC-Pharmacist Intern - STATE_MI State of Michigan Upon Hire required
  • LIC-Pharmacy Technician - STATE_MI State of Michigan Upon Hire required
  • CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required
  • CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required
  • This position will entail 6 months of On-Site work at Butterworth Hospital in Downtown Grand Rapids, Michigan
  • Once fully trained, you may have the opportunity to be scheduled for some Remote Shifts, while still residing in Michigan
  • Additional criteria may apply to work Remotely, such as being in good standing, being fully trained, internet capability at home, etc.
  • No scheduled Remote Shifts
  • Flexibility to pick up from the Open Remote Shift List

Nice To Haves

  • Preferred Associate's Degree health care or related field
  • 2 years of relevant experience Retail Pharmacy Preferred
  • Ideal candidate will possess Full Pharmacy Technician License and National Certification, such as PTCB Certification or ExCPT
