A Medication Historian is responsible for accurately collecting and documenting a patient’s medication history upon admission or during transitions of care. This role is critical for ensuring patient safety and supporting clinical decision-making. Work may include complex sterile and non-sterile compounding, program & quality assurance activities and / or billing. Potential for mentoring or precepting.
