About The Position

A Medication Historian is responsible for accurately collecting and documenting a patient’s medication history upon admission or during transitions of care. This role is critical for ensuring patient safety and supporting clinical decision-making. Work Focus: Performs pharmaceutical care activities (direct or indirect) for patients, clients, residents, participants, customers, hereafter referred to as “patients”, within the “scope of practice” laws & training received; Support pharmacy operations including medication preparation, distribution, procurement, and administrative activities (e.g., maintaining records or supplies) that enhance or improve medication management, improve safety, and decrease risk. Process Focus: Understands, articulates, & demonstrates full compliance in performing assigned duties following established policies & protocols. Communication: Uses clear, effective & respectful written, verbal & nonverbal communications with caregivers, colleagues, patients, families & visitors. Environment: Performs work in a caring, collaborative, safe & healthy manner; Influences the responsible use of resources; Accountable for continuous self-development & supporting the growth of others. Maintains a Working Knowledge of applicable Federal, state & local laws / regulations, Trinity Health Integrity & Compliance Program & Code of Conduct, as well as other policies, procedures & guidelines in order to ensure adherence in a manner that reflects honest, ethical & professional behavior & safe work practices. Functional Roles (not inclusive of titles or advancement career progression) PS II – Advanced: Expanded knowledge and experience of the PS I – General role. Work may include complex sterile and non-sterile compounding, program & quality assurance activities and / or billing. Potential for mentoring or precepting. Willingness to take on new or additional responsibilities appropriate for role and competence.

Requirements

  • High school diploma or GED or per state regulations as applicable
  • specialty license, registration, certification, training /or experience according to position or role and location of assignment in the applicable state of engagement.
  • Some local travel to different locations may be required.
  • Valid driver’s license where required by assignment.
