About The Position

The Medication Aide at Monroe Post Acute Services plays a crucial role in assisting with medication administration to residents in a hospice setting. This position ensures compliance with care plans and regulatory standards while providing quality care to subacute and chronically ill residents. The aide works under the supervision of a licensed nurse and is responsible for maintaining medication inventory, monitoring resident conditions, and ensuring safety and hygiene standards are met.

Requirements

  • High School diploma/GED or 10 years of work experience.
  • One year of geriatric experience.
  • State of Work Location: Wisconsin.
  • Basic Life Support HealthCare Provider (BLS HCP) certification from American Heart Association (AHA).
  • Medication Aide Training certification from TMU Wisconsin.
  • Certified Nursing Assistant (CNA) certification from Wisconsin Department of Health Services or Wisconsin Nurse Aide Registry.

Responsibilities

  • Maintains accurate inventory of medications at the beginning and end of each shift.
  • Ensures accurate distribution of medications to all residents and completes the 3 checks prior to administering meds.
  • Monitors for medication side effects and ensures medications are provided according to manufacturer's guidelines.
  • Provides medication pass assistance twice per shift.
  • Provides quality care to subacute/chronically ill residents and assists in the care of acutely ill residents.
  • Administers medications according to standards of care and maintains competency in all required skills.
  • Uses equipment according to standards of care and policy/procedure, reporting repair needs as necessary.
  • Monitors resident condition and reports significant changes to the team nurse.
  • Answers call lights and assists with two-person transfers, ambulation, range of motion exercises, and meal service.
  • Participates in report at the beginning of each shift and communicates changes of condition or unusual symptoms immediately.
  • Completes all assigned documentation requirements including medication/treatment administration.
  • Maintains privacy and confidentiality for residents at all times.
  • Monitors to ensure preventative measures specified on the residents' personalized care card are in place.
  • Responds promptly to all alarms and ensures safe room setup before leaving resident's room.
  • Treats pain and restlessness and assists with hygiene needs for residents at end of life.
  • Complies with safety policies/procedures to ensure safe practices for residents, staff, and self.
  • Uses and disposes of sharps according to safety standards and reports safety incidents or concerns.
  • Complies with infection control standards during tour of duty.
