About The Position

The Medication Adherence Specialist supports Independence Health System’s value-based care initiatives by improving medication adherence performance across payer programs, including Medicare Advantage and Part D. This role focuses on identifying patients at risk for non-adherence, conducting outreach to address barriers to medication access and persistence, and coordinating adherence interventions in collaboration with pharmacies, providers, and internal care teams. The Medication Adherence Specialist plays a key operational role in closing medication adherence gaps that directly impact quality metrics, payer incentives, and patient outcomes. This position supports data driven performance improvement efforts through patient engagement, documentation, and coordination of refill and pharmacy services.

Requirements

  • High School Diploma, GED or higher level of education.
  • Minimum of 2 years of experience working as a Pharmacy Technician
  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
  • Act 34-PA Criminal Record Check from the PA State Police system

Nice To Haves

  • Pharmacy Technician Certification.
  • Five (5) Years of Experience working as a Pharmacy Technician.
  • Experience in Healthcare setting.

Responsibilities

  • Generate and manage worklists of patients at risk of falling below medication adherence thresholds (e.g., <80% PDC) using payer reports, EHR tools, and adherence platforms. Prioritize outreach based on measure impact and seasonality.
  • Conduct telephonic and electronic outreach to patients identified as non-adherent or at risk. Utilize standardized scripts to assess barriers such as cost, side effects, forgetfulness, transportation, or pharmacy access. Document outreach attempts and outcomes.
  • Implement approved interventions to improve adherence, including facilitating 90-day or 100-day supply conversions, medication synchronization, refill reminders, mail-order enrollment, and pharmacy consolidation when appropriate.
  • Monitor refill queues and medication renewal status within the EHR. Coordinate with prescribing providers and pharmacies to resolve refill gaps when refills are available and escalate cases requiring provider or pharmacist intervention per defined protocols.
  • Serve as a liaison between patients, community pharmacies, and primary care teams to ensure timely medication access and continuity of therapy.
  • Accurately document interventions, outcomes, and patient-reported barriers in the EHR and tracking tools. Support performance monitoring for statin, diabetes, and RASA adherence measures tied to payer programs.
  • Participate in focused adherence initiatives during high-risk periods (e.g., fall adherence drop-off) and assist with payer-specific campaigns aimed at closing gaps prior to measurement deadlines.
  • Provide operational insights and summary data to the Value-Based Performance team to support ROI analysis, workflow refinement, and leadership reporting on medication adherence improvement efforts.
