The Medicare Wellness RN (MWN) at Duke City Primary Care is responsible for conducting Medicare Wellness visits with patients, either in person or via telehealth. This role requires collaboration with healthcare providers to ensure proper documentation, review, and completion of patient care tasks. The ideal candidate will demonstrate compassion and effective communication skills, as well as the ability to exercise clinical judgment in patient care. The MWN will be well-versed in chronic disease management and preventative care, providing necessary education to patients regarding their health needs. In addition to conducting wellness visits, the MWN will perform patient chart reviews to prepare for visits and manage various clinical requests. Responsibilities include outreach to patients, scheduling appointments, and verifying preventative services and Medicare Annual Wellness visits. The MWN may also assist with other clinical duties such as managing referrals, responding to patient messages, and processing prescription refills. A key aspect of the role is to identify and address quality care gaps, working collaboratively with patients to ensure these gaps are closed effectively. The MWN will document all necessary information in the electronic health record to maintain compliance with quality care standards. The position requires punctuality and preparedness for each shift, as well as respectful communication with providers and patients. The MWN will review daily schedules to identify patients for wellness visits and relay any patient concerns to the appropriate providers. The role also involves scheduling visits through various platforms, including Zoom and Telemedicine, and conducting comprehensive wellness exams with the assistance of clinical staff. Additional administrative duties may include faxing, scanning, entering referrals, and ordering labs, as well as triaging chronic care management and remote patient monitoring as needed. Regular participation in staff meetings and quality improvement projects is also expected to enhance patient care.
