The Medicare Medical Assistant provides direct patient care across the clinic, supports Chronic Care Management (CCM), and works with the Chronic Care RN to schedule and coordinate Annual Wellness Visits (AWVs). Provides high-quality, patient-centered care under the direction of a Physician, Nurse Practitioner, or Physician Assistant.

Requirements

  • High school diploma or equivalent preferred
  • MA certification preferred
  • Experience in a similar role, preferably in healthcare or public-facing setting involving registration and computer based tasks.

Responsibilities

  • Assess patient needs, obtain vital signs (pulse, temperature, blood pressure, weight, height), and record information accurately in the medical record.
  • Prepare treatment and examination rooms for procedures and patient visits.
  • Perform laboratory procedures, including drawing and processing blood specimens as needed.
  • Administer immunizations and assist with patient care tasks as directed by the clinical team.
  • Collect quality measures and ensure follow-up care is completed according to treatment plans.
  • Work closely with Chronic Care RNs to identify patients due for Annual Wellness Visits and schedule appointments.
  • Assist with Chronic Care Management (CCM) activities to support patient care and wellness goals.
  • Interact professionally with patients to address questions, concerns, and provide guidance within scope of practice.
  • Maintain current knowledge of public health policies, clinic procedures, and best practices.
  • Observe and follow the policies and procedures of Grand River Clinics and Grand River Hospital District.
  • Demonstrate teamwork, professionalism, and a willingness to support colleagues and patients.
  • Performs a variety of tasks and other duties as assigned.
