Provider cares for patients, by providing outpatient, primary care services that focus on patient centric measures such as access to care, value aligned prescribing, diabetes care, cardiovascular care. Provider will lead and work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff to provide care to patients. The position requires regular communication and collaboration with others. Panel size will be kept small to ensure good access, reduced costs and reduced need for the Emergency Department. Orchestrate treatments, medications, and non-pharmacologic therapy plans in partnership with patient and their caregivers/family Promote population health management programs and services to patients 14-16 patients per day
