About The Position
The Care Management Social Worker at EvergreenHealth is responsible for maintaining an Emergency Department (ED) patient assignment and works in partnership with the RN Case Manager and other healthcare professionals to ensure continuity and coordination of care for patients and families. This role involves providing consultation to patients and families regarding social, environmental, psychological, financial, and cultural factors related to illness and hospitalization, supporting continuity in care management, and organizing all aspects of discharge planning. The position assumes a daily case load of a select population to provide clinical social work services and performs other duties as assigned.
