About The Position

The Medical Social Worker plays a crucial role in assisting patients and their families in developing effective continuing care plans as they prepare for discharge from the hospital. This position involves addressing personal and environmental challenges that may hinder a patient's recovery and ensuring they receive maximum benefits from medical care. The Medical Social Worker must possess a comprehensive understanding of growth and development across all age groups and be able to communicate effectively with patients and their families.

Requirements

• MSW or equivalent degree required.
• SW/ASW licensure preferred (LCSW).
• At least one year of experience working with medical patients, or clinical hours towards licensure may be accepted.
• Ongoing education in the field is required.
• Possess assessment skills and sound judgment.
• Effective communication skills and basic computer skills are necessary.
• Knowledge of community resources and networking abilities are essential.
• Ability to conduct orientation and in-service classes for staff.
• Must attain authorization to write 5150 holds within 90 days of hire.

Responsibilities

• Communicate with patients and their families to develop continuing care plans for discharge.
• Assist patients and families in resolving personal and environmental difficulties.
• Assess and interpret data reflective of the patient's status and age-specific needs.
• Document care and instruction consistent with hospital and professional standards.
• Collaborate with the Case Manager RN to develop post-acute care plans for patients with barriers to wellness.
