About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Medical Social Worker:

Our team is devoted to walking alongside patients and families during one of life’s most meaningful journeys — and we’re looking for a Hospice Social Worker (MSW) who shares that calling.
As a Medical Social Worker, you won’t just be “filling a role” — you’ll be a lifeline of support, helping patients and their loved ones navigate emotional, social, and practical challenges with warmth and understanding. You’ll bring comfort in moments of uncertainty, empower families with resources, and provide a listening ear when it matters most.

Requirements

• Master’s Degree in Social Work (MSW)

• Strong communication and listening skills
• A collaborative, team-focused spirit

Nice To Haves

• Experience in hospice, healthcare, or counseling preferred

Responsibilities

• Provide emotional support, counseling, and education to patients and families
• Guide families through difficult decisions and connect them with community resources
• Advocate for patient needs as part of an interdisciplinary care team
• Support patients’ wishes, dignity, and spiritual well-being
• Help ensure that no patient or family ever feels alone in their hospice journey
