About The Position

Overview: $2,000 Sign-On bonus for new-to-NCH hires Monday - Friday, 8am - 5pm Full-Time, Benefits Eligible This position provides care coordination with children and teens who are involved in the juvenile-legal system. These patients may be currently in the JIC or recently released and need assistance with re-entry. These children may have recently had a Behavioral Health Juvenile Justice (BHJJ) assessment and could have a range of charges (misdemeanor and/or felony). Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. Must maintain a current Ohio Social Work license. Job Description: Essential Functions: The Social Worker (BSW) functions as a member of an interdisciplinary team and provides care coordination services to patients and their families including: Screening and identification of patients. Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination. Assisting Care Coordinator in completion of Health Risk Assessment. Assisting Care Coordinator with the development of family-centered plan of care. Facilitating communication and collaboration amongst the healthcare team (Frequently) Developing patient’s/family self-management skills through education and resource provision. Performs ad hoc tasks as directed by Care Coordinator. Managing transitions between settings, caregivers and providers. Monitoring and follow-up across the continuum of care. Providing consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services. Participates in orientation and continuing education of staff and students as appropriate.

Requirements

  • BSW, required.
  • LSW, required.
  • Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position-specific requirements.
  • Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
  • Active BLS certification, required.
  • Ability to communicate (verbally and written) clearly convey information to team members, healthcare providers, and families with professionalism and tact.
  • Organization, time management, and prioritization skills.
  • Ability to work within a multidiscipline matrix, collaborate with other providers and agencies.
  • Ability to relate to diverse age and demographic backgrounds.
  • Ability to analyze complex situations, identify potential problems, and develop effective solutions.
  • Computer literate with working knowledge of Microsoft products (i.e. Outlook, Work, PowerPoint, Excel, and Teams).
  • Basic knowledge of healthcare terminology.
  • Electronic Health Record experience, required.
  • Working knowledge of Medicaid and other regulatory agency standards, required.
