We are seeking a compassionate and motivated Part-Time Medical Social Worker to join our home health team serving the Greater Las Vegas area. In this role, you will support patients and families in their homes by providing psychosocial assessments, care coordination, and ongoing support as part of an interdisciplinary healthcare team. Requirements

• Active Medical Social Worker license in the state of Nevada
• Master’s degree in Social Work (MSW)
• Minimum of 6 mo of supervised post-licensure experience in a healthcare setting
• Ability to pass a background check
• Legally authorized to work in the United States

Nice To Haves

• Home health experience preferred

Responsibilities

• Conduct psychosocial assessments and implement individualized care plans
• Provide ongoing support, education, and counseling to patients and families
• Assist patients and caregivers with self-care strategies and community resources
• Coordinate services with physicians, nurses, and other care team members
• Document patient progress and provide timely reports
• Advocate for patients to ensure continuity and quality of care
