About The Position

Assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional and physical health. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. Must maintain a current Ohio Social Work license.

Requirements

Bachelor’s Degree in Social Work required.
Current Social Work license (LSW) in Ohio required.
Valid Ohio driver’s license and proof of auto insurance as required by hospital policy.
Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
Active BLS certification.
Ability to communicate (verbally and written) clearly convey information to team members, healthcare providers, and families with professionalism and tact.
Organization, time management, and prioritization skills.
Ability to work within a multidiscipline matrix, collaborate with other providers and agencies.
Ability to relate to diverse age and demographic backgrounds.
Ability to analyze complex situations, identify potential problems, and develop effective solutions.
Computer literate with working knowledge of Microsoft products (i.e. Outlook, Work, PowerPoint, Excel, and Teams).
Basic knowledge of healthcare terminology.
Electronic Health Record experience, required.

Nice To Haves

Experience working in healthcare in a patient facing role, preferred.
Working knowledge of Medicaid and other regulatory agency standards, desirable.

Responsibilities

Provides care coordination services to patients and their families as a member of an interdisciplinary team.
Screens and identifies patients.
Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination.
Assists Care Coordinator in completion of Health Risk Assessment.
Assists Care Coordinator with the development of family-centered plan of care.
Facilitates communication and collaboration amongst the healthcare team.
Develops patient’s/family self-management skills through education and resource provision.
Performs ad hoc tasks as directed by Care Coordinator.
Manages transitions between settings, caregivers and providers.
Monitors and follows-up across the continuum of care.
Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services.
Participates in orientation and continuing education of staff and students as appropriate.
