About The Position

Are you a compassionate and dedicated Licensed Social Worker looking to make a meaningful impact in the lives of patients in their homes? We are seeking a qualified individual to join our home health care team, providing vital mental health services to individuals in their home. Key Responsibilities: Conduct psychosocial assessments for patients and families, identifying needs, barriers, and resources Develop and implement care plans addressing emotional, social, and practical needs Provide counseling, crisis intervention, and patient/family education as appropriate Collaborate with clinical, intake, and administrative teams to coordinate services and referrals Assist with discharge planning, patient advocacy, and connecting patients to community resources Maintain accurate EMR documentation of assessments, interventions, and outcomes Monitor patient progress and report relevant information to clinical leadership Participate in quality improvement initiatives and case reviews Perform other administrative or clinical duties as assigned

Requirements

  • Master’s degree in Social Work (MSW) required
  • Current state licensure as a Social Worker (or eligibility)
  • 2–3 years healthcare or home health experience preferred
  • Strong assessment, counseling, and communication skills
  • Knowledge of community resources, patient advocacy, and care coordination
  • Proficiency with Microsoft Office; experience with EMR systems preferred
  • Ability to work independently in a fast-paced environment
  • Availability during business hours with occasional after-hours/weekend coverage

Nice To Haves

  • LCSW preferred
  • 2–3 years healthcare or home health experience preferred
  • Proficiency with Microsoft Office; experience with EMR systems preferred
