About The Position

We are seeking dedicated and compassionate Medical Social Worker to join our team. As a Medical Social Worker at Family Life care, you will play a vital role in managing the care of our patients by providing compassionate and professional care in the comfort of their own home. FLC is proud to serve our entire community from children to senior citizens. You will have the opportunity to choose from various assignments, such as patient case manager, long-term care or assisting with providing advice, support and resources to individuals and families to help them solve their problems which may include challenges in the home and/or assist with community resources. FLC works with you to build your work schedule around your needs.

Requirements

  • Current Medical Social Worker license
  • Current CPR Certification
  • Driver's License and Vehicle Insurance
  • Ability to pass a Level II background check

Nice To Haves

  • Positive, approachable, and mentoring personality
  • Understanding of state and federal laws and regulations related to home health, especially Medicare and Medicaid
  • Experience with Medicare (OASIS), Medicaid and commercial insurance in the home health environment is desired
  • Strong interpersonal and communication skills, organizational abilities, and attention to detail
  • Embrace technology and have experience with industry software such as Electronic Medical Records (EMR) and Electronic Visit Verification (EVV) systems.
  • "Go the extra mile" personality

Responsibilities

  • Patient assessment, and coordination of care with other disciplines, physicians and care managers.
  • Treatment of home care patients in accordance with the plan of treatment and with company policies and procedures.
  • Provides services in a pleasant, professional manner to maximize positive outcomes and patient satisfaction.
  • Ensure that all clinical assessments, care plans, starting and resumption of care are completed with agency guidelines.
  • Must be deadline driven.
  • Responsible for compliance with all state and federal, laws, regulations, policies, and processes relevant to home health.
