Home Health Medical Social Worker
About The Position
We are looking for a compassionate and driven Part-Time Medical Social Worker to join our home health team in the Greater Las Vegas area. In this role, you will provide essential psychosocial support, care coordination, and advocacy for patients and their families in the comfort of their homes. You will work closely with an interdisciplinary healthcare team to help ensure patients receive the highest quality of care. Requirements
- Active Medical Social Worker license in the state of Nevada
- Master’s degree in Social Work (MSW) required
- Minimum of 6 months of supervised post-licensure experience in a healthcare setting
- Ability to pass a background check
- Must be legally authorized to work in the United States
Nice To Haves
- Home health experience preferred
Responsibilities
- Conduct psychosocial assessments and develop individualized care plans
- Provide counseling, education, and ongoing emotional support to patients and families
- Help patients and caregivers access self-care strategies and community resources
- Collaborate with physicians, nurses, and other members of the care team
- Document patient progress accurately and submit reports in a timely manner
- Advocate for patients to promote continuity, safety, and quality of care
