Requirements
- Master’s or Doctoral Degree in Social Work (CSWE accredited)
- Active Social Worker license
- 1+ year of healthcare social work experience
- Strong communication and interpersonal skills
- Ability to travel within the coverage area (50%)
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to lift, push, or pull up to 50–100 lbs
Responsibilities
- Assess patients’ psychosocial, financial, and environmental needs
- Complete initial and ongoing social work evaluations
- Support care planning and discharge planning
- Connect patients and families with community resources
- Communicate changes and concerns with care team and physicians
- Document visits and maintain compliance with care standards
- Provide counseling and guidance to patients and caregivers
