The Medical Social Worker provides medical social services to clients in their homes in accord a nce with physician orders and under the direction and sup e rvision of the Director of N u rsing or other app r opriate supervi s or.
Requirements
Masters Degree from a School of Social W o r k app r ov e d by the Council of Social Work Education.
Two years' experie n ce with at least one ye a r of experi e n ce in a h e alth care setting (ho s pital, clinic, rehabilitation center, etc.)
Position requires the ability to communicate well orally and in writing.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition; to perform and demonstrate client care; and, to assess a client's home that may not be wheelchair accessible.

Acts as con s ultant to the members of the health team; helps the team understand t h e social, e m otional and envi r on m en t al factors related to the client ' s health p r oble m
Helps clients to utilize the resources of their families and the community.
Provides rehabilitative and supportive ca s
Assists clients and their families cope with personal and environmental difficulties which might predi s pose them t o illness or might interfere with their ability to obtain maximum benefits.
Assists in the client's admission process to the Agency by performing the initial evaluation and assessment of the clien t 's psy c ho s ocial status as well as an assessment of the client's home and family to identify socioeconomic a nd emotional factors that will affect the treatment plan.
Assists in d e velop m ent and i m ple m entation of the interdisciplinary client care plan as it pertains to medical s o cial work.
Observes, records and reports changes in t h e client ' s e m otional and s o cial factors that affect the client ' s illness and his/her need for care as well as his/her response to treatment.
Con s ults with the attending physician c o ncerning alteration of the plan of t r eat m
Enters clinical notes into EMR within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and progress notes and other clinical record forms outlining the services rendered.
Evaluates the client's and fa m ily ' s responses to, and eff e ctiveness of, the m edical social work intervention.
C onfir ms , on a weekly basis, the scheduling of visits with the D i rector of N u rsing t o facilitate coordination of other staff visits.
Participates in Agency activities and co m m ittees that include staff development and in-service education when appropriate.

Assumes responsibility for s e lf-development by continually striving to imp r ove his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active particip a tion in p r ofessional and relat e d org a nizational m eetings, and individual research and reading.
Complies with Agency dress code policy.
Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests.
Maintains current TB testing (refer to Health Screening Policy).
