About The Position
SteppingStone Adult Day Health is seeking a dedicated Social Worker to join our team, which is the largest provider of adult day health care in San Francisco. Since 1983, we have been committed to helping seniors and disabled adults achieve independent living through a comprehensive range of health and social services. This position offers a unique opportunity for the right candidate to enhance their social work and management skills while serving our participants and overseeing the essential intake process. As a Social Worker, you will work under the supervision of the SteppingStone ADHC Program Director and collaborate with a multi-disciplinary team. Your primary responsibilities will include assessing and reassessing participants in our Adult Day Health Care (ADHC) program. You will develop individualized care plans, monitor their implementation, document progress, and make necessary revisions at least bi-annually. Additionally, you will provide counseling and support to participants and their caregivers, addressing issues such as behavioral management, caregiver stress, and coping strategies. You will also facilitate communication between participants, their caregivers, and community services, ensuring a cohesive approach to care. Reporting any suspected abuse to the appropriate agencies is a critical part of your role. You will monitor participant attendance and case mix, and may facilitate support groups as needed. This position is vital in ensuring that our participants receive the highest quality of care and support in a nurturing environment.
