About The Position

The Medical Social Worker position at Swedish Hospital is a part-time role requiring 16 hours of work over the weekend, specifically on Saturday and Sunday with two 8-hour shifts. The role involves providing comprehensive direct patient continuum of care and case management services, coordinating and referring patients to health and social welfare resources, and assisting with discharge planning in collaboration with the Continuum Care Manager or Care Navigator. The Medical Social Worker will function as an integral member of a collaborative and interdisciplinary team, supporting nursing services and other clinical and non-clinical ancillary services to ensure the quality of service delivery and the continuum of care for patients. Additionally, the role includes serving as a patient advocate.

Requirements

• LSW or LCSW license required.
• Master’s Degree from an accredited school of social work required.
• CPR BLS Certification required.
• Two years’ post-graduate experience preferred; healthcare and/or mental health experience preferred.
• Experience with Electronic Medical Record (EMR) platform preferred.
• Experience with MS Office.
