Medical Social Worker/Social Worker Assistant-Southern San Luis Obispo
About The Position
Central Coast Home Health & Hospice is a fast-growing, locally owned and operated organization with an excellent reputation throughout the Central Coast. We are now seeking a full-time Medical Social Worker or Social Worker Assistant to work with both Home Health and Adult Hospice patients in the Southern San Luis Obispo areas (5 cities- Lompoc). This role offers the opportunity to make a meaningful impact while enjoying autonomy and flexibility, supported by clinical supervision from an experience LCSW. Clinical supervision is available for candidate working toward licensure.
Requirements
- Bachelor's Degree in Sociology, Psychology, Human Development OR Master's Degree from a school accredited by the Council on Social Work Education
- Minimum of one-year social work experience in a healthcare setting, home health &/or hospice experience preferred.
- Understand the "hospice philosophy" and issues of death and dying
- Reliable vehicle to drive for work
- Current Driver's license
- Clean background & driving record
- Maintain auto insurance coverage
- CPR certified
- Act calmly and professionally at all times
- Strong organization and communication skills
- Work well independently and in a team environment
- High attention to detail and good follow up
