About The Position

The Medical Social Worker I (MSW) provides psychosocial support and care coordination from admission through discharge. This role works closely with patients, families, the Interdisciplinary Care Team, and external providers to ensure safe, timely, and cost-effective care. The MSW develops and monitors care plans, advocates for patients and families, facilitates discharge planning, and serves as a resource for problem-solving and continuous improvement. Shift: Day Shift, PRN/Casual.

Requirements

  • Graduate of an accredited college/university.
  • MSW required with specific course work related to social work.
  • Experience in an acute care medical setting is preferred.
  • Current Maryland License as a Licensed Master Social Worker (LMSW).

Responsibilities

  • Assess patient needs across physical, cognitive, psychosocial, and developmental stages; provide age-appropriate care for infants through geriatrics.
  • Develop, implement, and monitor individualized plans of care in collaboration with patients, families, and care teams.
  • Coordinate discharge planning, including referrals to specialty services, community resources, and third-party payers.
  • Advocate for patients and families, ensuring understanding of insurance and care processes.
  • Act as liaison with external providers, participate in care conferences, and ensure timely completion of discharge-related functions.
  • Engage in outreach, education, and committee participation to support hospital and departmental goals.
  • Maintain communication with patients, families, supervisors, and co-workers to support safe, efficient, patient-centered care.
