About The Position

The Medical Social Worker for Pediatrics and NICU at Advocate Children's Hospital is responsible for providing social work services to patients, families, and individuals. This includes conducting psychosocial assessments, determining needs, implementing appropriate interventions, and planning for discharge. The role requires collaboration with a multi-disciplinary healthcare team to ensure safe and timely coordination of care, advocating for patient involvement in care plans, and connecting patients with community resources. The position is full-time, primarily during the day shift, with some rotation to holidays.

Requirements

  • IL-LSW Eligible, LCSW REQUIRED issued by the State of Illinois/WI-Social Work Certification issued by the State of Wisconsin.
  • Master's degree in social work from a Council on Social Work Education (CSWE) accredited school.
  • 1 year of social work experience in healthcare.

  • Previous experience in Pediatrics and/or NICU inpatient settings.

Responsibilities

  • Provides social work services to patients, families, and individuals including psychosocial assessment, screening, determination of needs evaluation, appropriate interventions and follow up, and discharge planning.
  • Participates as part of the multi-disciplinary health care team to develop safe and timely coordination of care including post-acute placement, palliative/hospice service lines, and other community resources.
  • Maintains up-to-date knowledge of community resources, legislation, and regulations impacting health care delivery.
  • Communicates effectively with patients, family, other members of the health care team and community agencies.
  • Manages the progression of patients' stay with the goal of optimizing the length of stay and ensuring appropriateness of assigned level of care.
  • Provides education to patients/families regarding Advance Directives for health care decision-making.
  • Maintains professional standards and responsibilities for professional practice according to accreditation, hospital, system, state and NASW Standards and Code of Ethics.
