About The Position

At NorthBay Health, the Medical Social Worker - Hospital plays a crucial role in providing direct clinical social work services, counseling, discharge planning, and case management to hospital patients and their families. This position involves a comprehensive assessment and documentation of patients' emotional, social, financial, environmental, or follow-up referral needs. The Medical Social Worker collaborates closely with the interdisciplinary healthcare team to formulate a psychosocial plan of care tailored to the unique strengths and needs of each patient. The Medical Social Worker is responsible for developing a discharge plan for their designated caseload, monitoring progress toward care goals, and identifying any barriers that may affect the estimated length of stay. This role also facilitates the integration of community-based resources to optimize patient health outcomes. All activities performed by the Medical Social Worker are consistent with the System's mission and values and comply with applicable regulations and policies. To qualify for this position, candidates must hold a Master of Social Work degree from a school accredited by the Council on Social Work Education. While an MSW license is preferred, it is not mandatory. Candidates should have a minimum of two years of experience in an acute healthcare setting or community health setting, or have completed an internship or externship at NorthBay HealthCare. Strong psychosocial assessment and critical thinking skills are essential, along with the ability to work effectively with diverse populations, including those of varying ages, cultures, ethnicities, and genders. Proficiency in word processing and basic database skills is required, along with strong written and oral communication skills. The Medical Social Worker must demonstrate the NorthBay Way principles of Caring, Communication, Collaboration, and Competence in their interactions with patients and team members.
