About The Position
We’re seeking a compassionate and motivated Medical Social Worker (MSW) to join our Home Health team on a PRN basis in the Dublin, Ohio area. The MSW will evaluate and support patients and families dealing with medical, emotional, and social challenges — helping them access the resources and support needed for successful care at home.
Requirements
- Master’s degree in Medical Social Work (MSW) or equivalent license
- Minimum 2 years of experience in a medical or home health setting
- Current CPR certification
- Valid driver’s license and proof of auto insurance
- Ability to attend a 1–2 day orientation at our Dublin office
Responsibilities
- Provide psychosocial evaluation, diagnosis, and treatment planning
- Offer individual, family, and/or group therapy as needed
- Assess and address social or personal issues impacting patient independence and recovery
- Connect patients and families to appropriate community resources
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Maintain accurate, up-to-date documentation per HIPAA standards
- Provide written reports for patient records, including evaluations, progress notes, and discharge summaries
- Participate in family conferences and patient care planning
- Discuss care concerns and recommendations with the Director of Clinical Services
