About The Position

As a Medical Social Worker (MSW) with GrandCare East, you will provide psychosocial support to patients and families in their home environment, helping them navigate emotional, social, and resource-related challenges associated with illness.

Requirements

• Graduate of a master’s program in social work accredited by the Council on Social Work Education.
• 1+ year of experience in health care (home health experience preferred).
• Demonstrate good verbal and written communication, and organization skills.
• Possess and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Responsibilities

• Assess psychosocial needs related to the patient’s illness, home environment, emotional well‑being, and support systems, and communicate findings to the clinical team.
• Develop and implement social work intervention plans, conduct social evaluations and maintain comprehensive clinical documentation.
• Connect patients and families with community resources, providing information, referrals, and acting as a liaison with public and private agencies.
• Collaborate with the interdisciplinary team, contributing to care plans, case conferences, and helping clinicians understand social and emotional factors affecting health.
• Support care transitions, actively participating in discharge planning and performing additional duties delegated by nursing leadership.
