About The Position

CareCo Medical Home Health is seeking a licensed Medical Social Worker (MSW) to provide in-home social work services to patients in the community. This per diem role is an excellent opportunity for a social worker who enjoys working independently while collaborating with a multidisciplinary care team. Our services include nursing, therapy, and social services, all designed to help individuals remain safe, strong, and independent in their homes. We support patients with acute and chronic conditions, care coordination, and access to community resources. Position Summary The Medical Social Worker addresses the social, emotional, and environmental factors that impact a patients ability to achieve the goals of the Plan of Care. Services are provided under physician direction and in collaboration with nurses, therapists, and other care team members, following agency policies and professional standards.

Requirements

  • Masters Degree in Social Work from a program accredited by the Council on Social Work Education
  • Minimum of three years of experience in a healthcare setting or equivalent experience
  • Strong written, verbal, and interpersonal communication skills
  • Knowledge of community resources and social service systems
  • Valid drivers license, reliable transportation, and current auto insurance

  • Home health experience preferred

Responsibilities

  • Assess the psychosocial status of patients and identify barriers to care
  • Complete assessments and documentation in a timely manner
  • Develop and implement care plans in collaboration with the interdisciplinary team
  • Provide counseling, guidance, and support to patients and families
  • Educate patients, families, and team members on available community resources
  • Initiate referrals and assist with discharge planning
  • Participate in care conferences, team meetings, and quality improvement activities
  • Communicate changes in patient status to physicians and care team members
  • Maintain compliance with agency, state, and federal standards
