About The Position

Our Medical Social Workers play a vital role in helping patients and families navigate complex life situations with dignity, compassion, and professional guidance. We are currently seeking a dedicated Medical Social Worker (MSW) to join our San Ramon-based home health team. In this role, you will provide psychosocial support, counseling, and care coordination for homebound patients, ensuring they receive the resources and services needed to maintain independence and well-being.

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited program
  • Minimum 1 year of clinical experience (home health or hospice preferred)
  • Current CPR certification
  • Valid California driver’s license, reliable transportation, and active auto insurance
  • Strong communication, advocacy, and time management skills

Nice To Haves

  • LCSW preferred, but not required

Responsibilities

  • Conduct comprehensive psychosocial assessments and develop individualized care plans
  • Provide guidance regarding advance directives, long-term care planning, and end-of-life decision making
  • Assist patients and families during crisis situations and transitions in care
  • Connect patients with community resources, including transportation, financial assistance, housing, and food support
  • Provide short-term counseling and emotional support to patients and caregivers
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure holistic care
  • Participate in interdisciplinary case conferences and care planning meetings
  • Maintain accurate documentation in EMR (Axxess) while ensuring compliance with CMS and Title 22 regulations
