Requirements
- Certified Social Worker - Required
- 2+ years of MSW experience in Home Health or Hospice setting - Required
- Experience in Hospice or palliative care - Required
- Strong interpersonal and counseling skills.
- Valid Drive License and reliable transportation.
Responsibilities
- Assess patients’ and families’ psychosocial needs and develop care plans.
- Provide counseling and emotional support to patients and caregivers.
- Assist families in accessing community resources, financial aid, and legal support.
- Collaborate with the interdisciplinary hospice team to ensure holistic care.
- Maintain accurate and timely documentation in compliance with regulations.
