About The Position

The social worker provides social work services to patients and their families in order to enhance psychosocial functioning and continuity of care. The social worker conducts psychosocial assessments of patients/families. The social worker, in collaboration with the multidisciplinary team, develops and implements support for identified patients. The social worker provides psychosocial intervention with trauma victims, patients with psychosocial disorders, and patients having a difficult time adjusting to an illness as applicable.

Requirements

  • Education: Master’s Degree in Social Work
  • Required Experience: Experience in healthcare setting
  • Special Skills and Abilities: Requires the ability to write information and to enter/retrieve information from a computer. Requires the ability to speak clearly and to hear conversations when there is background noise. Requires the use of universal precautions due to exposure to blood-borne pathogens. Work involved mobility throughout the hospital.
  • Licensure/Certification/Registration: BLS Certification. Valid driver’s license and proof of vehicle insurance

Responsibilities

  • Conducts the psychosocial assessment of the client and participate in the development of the plan of care.
  • Assist the physician and other members of the health care team in understanding the social and emotional factors related to the client's health problems.
  • Assess the social and emotional factors in order to estimate the client's capacity and potential to cope with the problems of daily living.
  • Help the client and family understand, accept and follow medical recommendations and provide services planned to restore the client to the optimum social and health adjustment within the client's capacity.
  • Assist client and family with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
  • Utilizes all available resources, such as family and community agencies, to assist the client to resume life in the community or to live within the disability.
  • Observe, record and report social and emotional changes to appropriate personnel. Prepares clinical and progress notes for the clinical records.
  • Participates as a member of clinical record/QI when applicable.
  • Attends and participates in multi-disciplinary conferences.
  • Documents clinical progress notes according to Medicare standards.
  • Displays competency for age specific patients; newborn through geriatrics.
  • Supervises and oversees the work of the social work assistant on a regular basis.
  • Other duties may be assigned.
